Wisdom teeth - to extract or not to extract?

Wisdom teeth are the last molars to appear, usually in the late teens or early twenties. For some people they arrive quietly and function like any other tooth. Some erupt without issue, but many cause pain, infection, or crowding that makes wisdom teeth removal the healthier choice.

We’re often asked: “Do I really need my wisdom teeth taken out?” The truth is, not every wisdom tooth needs to be extracted. At Eve Dental, we assess each patient individually and plan treatment to ensure a safe, comfortable, and pain-free extraction when needed.

Why do wisdom teeth cause problems?

Modern jaws are often smaller than those of previous generations, leaving limited space for these last molars. When there isn’t enough room, wisdom teeth can become trapped or grow at awkward angles, leading to a range of issues.

The most common reasons we recommend extraction include:

1. Dental Pain

Persistent pain at the back of the mouth is often the first sign of trouble. Wisdom teeth can press on neighbouring teeth, nerves, or bone, causing aching or sharp discomfort. If pain keeps returning despite good cleaning and pain relief, removal is usually the most reliable long-term solution.

2. Decay That Can’t Be Restored

Because wisdom teeth are so far back, they’re difficult to clean properly. Food and bacteria easily become trapped, leading to cavities. Sometimes the decay is too deep or positioned in a way that makes fillings unpredictable. In these cases, extraction is safer and more predictable than complex repairs.

3. Infection

Partially erupted wisdom teeth create pockets where bacteria thrive. This can lead to swelling, bad taste, difficulty opening the mouth, and sometimes fever. Recurrent infections are a clear sign that wisdom teeth removal is the safest option.

4. Gum Irritation and Recurrent Inflammation

Even without full infection, wisdom teeth can constantly irritate the surrounding gums. Redness, swelling, and soreness may come and go, especially during stressful periods. Ongoing inflammation can affect the health of neighbouring teeth and your overall oral wellbeing.

5. Impaction – Not Enough Room to Erupt

When there isn’t enough room in the jaw, wisdom teeth become impacted—growing sideways, tilted, or trapped under the gum. Impacted wisdom teeth can damage adjacent molars and are one of the most common reasons people need wisdom teeth removal.

6. Pathology – Cysts and Other Changes

In some cases, impacted wisdom teeth are linked to cysts or changes in the jawbone. These conditions may develop silently and can weaken bone if left untreated. Early assessment at our Ryde & Macquarie Park dental clinic helps prevent more serious problems.

Can wisdom teeth stay?

Not all wisdom teeth need to be removed. If a tooth has fully erupted, easy to clean, pain-free, and not damaging other teeth, we may simply monitor it with regular check-ups and X-rays. In some cases, we use laser technology to restore teeth that are partially covered by gum tissue.

At Eve Dental , we prioritise early assessment of wisdom teeth. Reviewing wisdom teeth in the late teens or early twenties—when the roots are less developed and the bone is more flexible—allows us to diagnose potential problems before they occur.

Early planning often means simpler procedures, faster healing, and a lower risk of complications, leading to better long-term outcomes for our patients in Ryde and across Sydney.
